Adult Degenerative Scoliosis

Case Information

66 year old female with progressive low back pain and spinal deformity and bilateral leg pain that gets worse with ambulation.
Physical therapy, epidural injections, acupuncture, and chiropractic manipulations failed to provide relief and the patient did not want to live with the pain any longer.
X-rays showed a Lumbar curve with a fractional curve at the lumbosacral junction, and loss of lumbar lordosis and thoracic kyphosis.
MRI revealed L2/3 and L3/4 stenosis
A combined anterior/posterior procedure on one day was performed.
First, the discs and ligaments from the front of the spine were released to increase the mobility of the spine from T12-L5. Bone chips from a tissue bank and bone morphogenetic protein were placed to promote fusion.
Then from the back, the lamina were removed from L2-L4 to decompress the spinal cord, then an instrumented fusion from T9 to the pelvis was performed with allograft, autograft, and bone morphogenetic protein.
