Gupta, M.D. Category Adult, Revision Surgery, Spinal Deformity Case Information 43 year old female with previous L4-S1 posterior spinal fusion with back and left leg pain. Patient had congenital scoliosis. Failed injections and physical therapy. Clinically, she had severe sagittal imbalance, and imaging showed loss of lumbar lordosis, thoracic kyphosis and rod failure. Removal of instrumentation and posterior spinal fusion from T-10 to the pelvis was performed with a pedicle subtraction osteotomy (PSO) at L4. Bone from a tissue bank, the patient’s own bone, and bone morphogenetic protein were used to promote fusion. Postoperatively, she had improved ability to stand upright clinically, and improved sagittal alignment on x-ray.
